My future care: an introduction to Advance Care Planning

By making plans that cover your future health care, you’re not only working out what you want, you are also making it easier on those around you.

An Advance Care Plan enables you to make some decisions now about the health care you would or wouldn’t like to receive if you became seriously ill and were unable to communicate your preferences or make treatment decisions. This free workshop, presented by Palliative Care WA, will cover information about what’s involved in Advance Care Planning, how to get started, and how to talk about your wishes for the last stage of your life with your family, loved ones and health care providers.

This small group workshop is facilitated by Palliative Care Western Australia for people living with a metastatic breast cancer diagnosis. Minimum numbers will be required for this workshop, so please register as soon as possible.
